You receive both once you reach level 220. So it may have been a visual or or something if you are not level 220.
When you are not at level 220, and even i've experienced this myself. At random (it can/cannot happen) you will be randomly wearing one of the two visuals on your survivors head whenever you join the game. also the type of visual you wear on your survivors head is also considered to be at random. This feature/something has been in mgftw since V1.5.2.0 level 220 update.
